Forms and when they are due:

Change of CONTACT Info Form – Mandatory only IF your contact information changes.

Changes to basic contact info are free. Please be sure to keep us up to date with your most current info.

Change of SHOW Info Form – Mandatory only IF your show information changes – fees may apply.

If you are changing show information such as Company Name, Show Name, Show Description, etc. a fee is required in order for your change to be processed. The following fees will be due depending on when you fill out the form:

From date of application – date Listing/Production Fees are paid: FREE
From date Listing/Production Fees are paid – June 1st: $30.00
From June 1st – June 15th: $75.00
After June 15th: No changes permitted.

Withdrawal Form - Mandatory only IF you decide to withdraw.

We are sorry to see you go! If you are having doubts or have any questions before you withdraw, please don’t hesitate to contact us. Maybe we can help!

Please be aware of these guidelines regarding refunds:

- on or before Feb. 16, 2012: 100% refund of Listing and Production Fees
- on or before February 23, 2012: 90% refund of Listing and Production Fees
- on or before April 3, 2012: 50% refund of Listing and Production Fees
- after April 3, 2012: no refund
